We recommend printing and completing these forms at your convenience before your child's first appointment. If you have any problem getting these files to print properly, please call our office and we will gladly send or fax them to you.
At Healthy Smiles for Kids, we understand that you have busy schedules to coordinate. That's why we offer convenient day and evening appointment times that are suitable for both you and your child. Our office hours are Monday - Friday 8:00 a.m. - 5:00 p.m. We welcome you to call us at (903) 531-9000 to reserve your appointment time!
Because all children are more prepared and do better in the mornings, we try our best to schedule any restorative work such as fillings and extractions during the morning hours. School-age children will be given a note from our office regarding their dental appointment time so that their absence will be excused.
We are excited to offer your child exceptional pediatric dental care during a time that is specifically set aside just for them. We strive to give every child the time and individual attention they deserve. In a sincere effort to do this, and to remain on time during our busy schedule, we ask you to ARRIVE ON TIME FOR YOUR CHILD'S APPOINTMENT. This allows us to be able to see all the children that are scheduled thoroughly, timely and in an efficient way. Please plan to arrive 5 minutes or more before your appointment time to complete any additional paperwork so that your child may be seen on time.
If you feel that you may be more than 10 minutes late for your appointment, please call right away. We may still be able to see your child or we may need to reschedule the appointment at a later date. If we are able to see your child, we cannot guarantee that all treatment will be completed.
We understand that unexpected things happen causing a change in your schedule. So, as a courtesy, we ask that you notify us 24 hours in advance of your scheduled appointment time if you are unable to keep it. If a 24-hour notice is not given, a $25 fee will be charged to your account. If more than two broken/missed appointments or two cancellations without 24-hour notice occur, we reserve the right to refuse scheduling any subsequent appointments.

If your child requires any dental treatment, we will provide you with a detailed treatment plan and we will explain to you all estimated costs. Because we want you to be fully informed of proposed treatment plan, please feel free to ask us any questions you may have. We want you to be confident that the treatment is the best course of action for your child's dental and overall health. We will not proceed with any treatment until we have written consent from a child's parent or legal guardian.
As a courtesy to you, our office will file dental insurance claims with your insurance company. We accept:
We request that you provide our office with accurate information relative to your insurance plan prior to treatment. That way, we can assess coverage to help you best maximize your insurance benefits. Please know that you are ultimately responsible for any services that are not covered by your insurance company.
Payment is due the day services are performed. We accept cash, check, debit cards and most major credit cards. If you need further assistance, we are happy to offer CareCredit Financing. CareCredit is a convenient, no interest low minimum monthly payment plan for your entire family designed to pay for healthcare charges not covered by insurance. If for any reason your account becomes 90 days past due, it may be turned over to our Collection agency. You will be responsible for all costs of the collection process.
